Henoch-SchöNlein Purpura in Adults

چکیده
Henoch-Schönlein Purpura (HSP) is a systemic vasculitis mediated by IgA and characterized by the clinical triad of non-thrombocytopenic palpable purpura, abdominal pain, and arthritis. In addition, there may be varying degrees of renal involvement. It is the most frequent vasculitis in children and the incidence in adults varies from 3.4 to 14.3 cases per million, depending upon the experimental series analyzed. We report the onset, course and response to treatment of five adult patients who required hospitalization at our institution between 2002 and 2006 (all fulfilled the 1990 American College of Rheumatology (ACR) criteria).

Abnormal IgA glycosylation in Henoch-Schönlein purpura restricted to patients with clinical nephritis.
BACKGROUND Glomerular deposition of IgA1 is a common feature of Henoch-Schönlein purpura, and is indistinguishable from that seen in IgA nephropathy. Serum IgA1 is abnormally O-glycosylated in IgA nephropathy, and this may contribute to mesangial IgA1 deposition and the development of glomerular injury. This altered O-glycosylation of IgA1 can be detected by its increased binding to the lectin ...

Henoch-Schönlein purpura is an acute, systemic, immune complex-mediated, leukocytoclastic vasculitis. It is characterized by a triad of palpable purpura (without thrombocytopenia), abdominal pain, and arthritis. Most patients have an antecedent upper respiratory illness. More than 90 percent of Henoch-Schönlein purpura cases occur in children younger than 10 years; however, adults with this con...
متن کاملBullous presentation of Henoch-Schönlein purpura in an adult.
Henoch-Schönlein purpura is a small vessel vasculitis occurring mainly in childhood and rarely in adulthood. Typical cutaneous eruption may begin as macular or urticarial erythematous lesions progressing to a palpable purpura. In adults, the disease has a propensity to be more severe and chronic and affects mainly the ankles and feet. Bullae, vesicles and ulcers are occasionally seen in this gr...
متن کاملSuccessful treatment of severe gastrointestinal involvement in adult-onset Henoch-Schönlein purpura.
Henoch-Schönlein purpura is a small vessel vasculitis which is uncommon in adults. The presentations of adult-onset disease are different from those seen in childhood. The commonly-recognised serious gastrointestinal complications of childhood are less well-documented in adults. We report three cases of adult-onset Henoch-Schönlein purpura with severe gastrointestinal involvement. All were men,...
متن کاملAn unusual case of Henoch-Schönlein purpura.
Vasculitis has protean manifestations with etiological non-specificity of histological lesions. The natural history of Henoch-Schönlein purpura in adults is less well established. We report an adult female patient who presented with palpable skin purpura and was evaluated to have IgA nephritis who remains stable on treatment with cyclophosphamide and steroids.

Received: 18 Aug, 2008 Accepted: 14 Feb, 2009 Abstract Renal involvement is one of the most serious sequela of Henoch-Schönlein purpura. The presence of proteinuria (nephritic range) and hematuria is also associated with progression to renal insufficiency. In fifty percent of patients who display a combination of nephritis-nephrotic symptoms, end-stage disease develops. Pharmacologic treatme...
